We are required by law to maintain the privacy of your Protected Health Information ("PHI"), follow the terms of this Notice, and notify you in case of a breach of unsecured PHI.

Confidential Communications: You have the right to receive communications about your PHI through alternative means or at different locations, upon request. Access to PHI: You may access, inspect, and obtain a copy of your PHI, including electronic records. We may charge a fee for copies and may deny requests in limited situations. Request Amendments: If you believe your PHI is incorrect or incomplete, you can request an amendment. We will review and respond to your request but may deny it in some circumstances. Accounting of Disclosures: You can request a list of disclosures of your PHI made by us over the past six years, free once per year. Additional requests may incur a fee.
Paper Copy of This Notice: You can request a paper copy of this Notice at any time, even if you received it electronically.
Opt-Out of Fundraising: You have the right to opt-out of fundraising communications. Your PHI will not be used for fundraising or sold without your prior authorization.
